Awkward rust - do I need to weld?

You can see on the passenger side it rusted through the backside panel as well as onto part of the door receiver frame (what's the term for that?)
Those frame support bars add another obstacle since I can't slide any material behind the panels.
No idea how to approach this - mesh, sheet metal, or some kind of porous filler?
Am I going to need to cut out above and around those support bars to properly sand and seal the rust?

Pretty new to this stuff, any suggestions appreciated. Thanks guys.
